Appointments

- Press 1 for Urgent Medical Problems that requires immediate physician attention. Available 24 hours a day
- Press 2 for Same Day Sick appointments
- Press 3 for Non-urgent Questions for the nurse or doctor regarding general care for your child.
- Press 4 for Routine Well, Newborn, or Prenatal Consult appointments or if you have a General Question for the staff
- Press 6 for Test Results, Refills or Medical forms
- Press 7 for General Information including location and hours
- Press 8 to Request Medical Records
- Press 9 to speak to the Office Manager or with Billing or Insurance questions
Office visits are by appointment only. Please give at least 24 hours notice when canceling or rescheduling an appointment to avoid being charged for the visit. This will allow someone else to take advantage of that appointment time. Appointments made before or after office hours are assessed an after-hours fee in addition to the regular office visit fee.
In order to see our patients in a timely manner, we cannot see walk-ins or siblings without an appointment. As a courtesy to our other patients, please call if your child needs to be seen.
Please note that hours may vary according to holiday schedules and physician availability.
Please arrive 15 minutes early for your appointment. We are unable to maintain our schedule during the course of the day if patients arrive late for their appointments. Patients who are late may be required to reschedule their appointments. If you are going to be late, please call us so that we may try to rearrange our schedule. Every effort will be made to accommodate you.
Relatives and Caretakers
We understand that a parent may not always be able to come to your child's appointment. If your child will be accompanied by a caretaker other than a parent, please fax or send along a written, signed letter or our release form (
click here for forms) giving permission to our staff to examine and administer vaccines or medications to your child in your absence. We are also required by law to have written permission to disclose any pertinent medical advice or information to a relative or caretaker via the telephone. Please note that we will not be able to render care without written permission under federal HIPPA guidelines.
At each visit, you will be asked to document if you have had a change in address, phone number, or insurance information. If this has changed between visits, please let us know. If your insurance information has changed, we ask that you bring a copy of your card to the visit. If you don't have proof of insurance, you will need to pay at the time of the visit, and you may submit it to insurance for reimbursement.
All new patients and patients less than 18 years of age must be accompanied by a parent or authorized adult guardian in order to be evaluated by a clinician.
